CDC changes recommendations on separation of mother and baby:
On April 4, the CDC made revisions to their February 18 Interim Considerations for Infection Prevention and Control of Coronavirus Disease 2019 (COVID-19) in Inpatient Obstetric Healthcare Settings. A revision was made to reflect that the decision of whether to keep a mother with known or suspected COVID-19 and her infant together or separated after should be “on a case-by-case basis, using shared decision-making between the mother and the clinical team.”

Update: This is an answer to an email we received asking if we had changed our recommendations on mask use in our webinar of March 24th.
“Recommendations have changed and midwives should wear facemasks, as should patients and family members. At this time it is recommended that medical masks such as N95 be used only with sick patients. But home made facemasks are now recommended for everyone. One new thing we did not know two weeks ago is that this virus may be very contagious when an infected person has no symptoms, or is in the early stages and will not get sick for a few more days.
And please be aware that all advice we give is only good until the next update... this is called a "novel" coronavirus, because we just don't know enough yet so we have to be very flexible to change with the latest recommendations! Be safe”

Vicki was glad to comply and hopefully give her fellow midwives in America a better understanding of what is needed now.

A MODEL FROM THE STATES OF ALASKA ON CORONAVIRUS PREPAREDNESS
Alaska Family Health & Birth Center in Fairbanks, Alaska, has written some wonderful documents that they are allowing us to share here. Two clinkable PDFs are below. (Our Adjunct professor Manga Penwell is a staff midwife, and Vicki Penwell was the original founder in the 1980s) We are very proud of AFH&BC for their compassionate and forward-thinking response to the increased need/desire for pregnant women to make a late transfer of care. The second document is part of the collaboration with local health care providers and hospitals, which addresses the understanding that it may become crucial for midwives to care for most of the normal births in a community should the hospital become overwhelmed by the COVIC-19 pandemic.

From the CDC about hand-sewing Face Masks:
HCP use of homemade masks:
In settings where facemasks are not available, HCP might use homemade masks (e.g., bandana, scarf) for care of patients with COVID-19 as a last resort. However, homemade masks are not considered PPE, since their capability to protect HCP is unknown. Caution should be exercised when considering this option. Homemade masks should ideally be used in combination with a face shield that covers the entire front (that extends to the chin or below) and sides of the face.

CDC Recommendations to Prevent Spread of Disease
Stay home when you are sick (and until your symptoms are gone).
Wash your hands often with soap and water for at least 20 seconds. If soap and water are not available, use an alcohol-based hand sanitizer.
Avoid touching your eyes, nose and mouth with unwashed hands.
Avoid close contact with people who are sick.
Cover your cough or sneeze with a tissue, then throw the tissue in the trash. If a tissue is unavailable, use the inside of your elbow, never your hand.
Clean and disinfect frequently touched objects and surfaces.
If you have a fever (100.4°F/38°C or higher), cough or have trouble breathing:
Call ahead before you go to a doctor’s office or emergency room.
Tell your doctor or state health department about your recent travel and your symptoms.
Avoid contact with others.
